A local healthcare provider in Waddington seeks an Advanced Nurse Practitioner for a 15-hour/week role on a 12-month fixed contract. The ideal candidate will provide autonomous healthcare, managing patients' clinical care within a multi-disciplinary HomeHealth team. Responsibilities include assessing undiagnosed conditions and delivering high-quality patient-centered care, requiring flexibility in work hours. This is an exciting opportunity for an Advanced Nurse Practitioner to join South Lincoln Healthcare PCN in a temporary capacity as part of our exciting and innovative HomeHealth Team.
This role is for 15 hours per week, on a 12 month fixed term contract.
The post holder will use specialist knowledge and skills to provide healthcare autonomously, being accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. They will practice at an advanced level, demonstrating in‑depth knowledge and competence in all aspects of frailty, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence‑based and deliver high quality patient‑centred care.
The post holder will work within and across designated care homes for patients registered at our 7 member practices:
The role will support the PCN’s passion for providing an all‑encompassing approach to personalised care and health inequalities across the PCN.
The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.
