Job description
Overview

Leadership in patient safety across the patient safety team, directorate and clinical teams in all aspects of patient safety. The post requires expertise in Patient Safety and reviews, including Systems Engineering Initiative for Patient Safety (SEIPS) and Human Factors. The role involves working flexibly as part of the wider Patient Safety team and may require travel to different sites across the geographical area.

Responsibilities
  • Provide leadership in patient safety across the patient safety team, directorate and clinical teams in all aspects of patient safety.
  • Be an expert in Patient Safety and reviews, techniques and practice, including SEIPS and Human Factors.
  • Monitor and interpret national guidance and policy to ensure that Trust procedures and practice comply with or exceed best practice.
  • Review patient safety incidents and/or complex complaints, acting as a role model and expert advisor for staff involved in the reviews, ensuring reviews are carried out to the highest standards.
  • Produce high quality reports identifying the system and process factors that led to incidents/complaints, and suggest safety improvement areas for change in practice.
  • Analyse themes and patterns from incidents and complaints to support learning and changes; present these findings in internal and external forums.
  • Share learning across the Trust and deliver patient safety training.
  • Work flexibly as part of the wider Patient Safety team, undertaking duties relevant to the post to improve the experience of patients and staff.
  • Travel to different sites across the geographical area as required.

About the role and team

The team works across all directorates within the Trust, including community, dental, mental health, forensic and learning disability services. The role provides exposure to a variety of services and incident types, with some incidents (e.g., self-harm/suicide-related) being more prevalent due to service nature. The Trust operates across Oxfordshire, Buckinghamshire, Swindon, Wiltshire, Bath and Northeast Somerset.
