Vacancy Overview

A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust.

Role Overview

Work as a senior member of an established Community Mental Health Team in the East Locality of Doncaster, providing a high standard of safe, robust and evidence based clinical practice.

Work with complex organic and functional diagnoses within the community setting, liaising and integrating with both statutory and voluntary organisations in support of the specific client group.

Responsibilities
  • Promote and maintain the highest attainable standard of nursing care in a community setting.
  • Support the band 6 nursing team with implementation of evidence‑based interventions from a plan of care.
  • Perform re‑assessments of clients to include risk and crisis plans.
  • Maintain close working relationships with referring agencies.
  • Deputise for the Team Leader when required.
  • Carry a caseload of patients with both functional and organic issues as identified through the team systems process.
