  • Provide preventative support to promote independence and be responsible for the assessment, development, review, and monitoring of individual care plans.
  • Assess needs for adaptations, equipment, and moving and handling to facilitate independent living.
  • Work collaboratively with housing needs to ensure suitable living arrangements for clients.
  • Utilise your Front Door Service duty experience to contribute effectively to the team.
  • Drive to various locations as required; having a car is essential for accessing peripheral stores, and transporting and installing small aids and equipment to support assessment purposes.
Requirements

  • Degree or equivalent qualification in Occupational Therapy.
  • Proven experience in the UK/NHS or hospital as an Occupational Therapist.
