Job Title

Band 5 Staff Nurse

Overview

You will provide skilled nursing care and work flexibly with the team to deliver high quality integrated care. Your day will involve holistic assessment, enabling patients to receive care in their homes and clinic settings, and preventing unnecessary hospital admissions.

Responsibilities
  • Assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patient, carers and other professionals.
  • Deliver evidence‑based interventions.
  • Be responsible for the safe monitoring, management and administration of medication.
  • Work autonomously, building leadership and advanced clinical assessment skills.
  • Build effective relationships with patients, carers, families and MDT colleagues.
  • Develop exceptional planning and organisational skills.
  • Analyse care and treatment provided, and develop sound clinical decision‑making.
  • Participate in training to deliver high quality care in areas such as palliative care, leg ulcer management, acute and chronic wound care, bladder and bowel dysfunctions, and supportive care to carers/families.
  • Delegate and support junior staff, further building leadership skills.
Qualifications
  • Registered adult nurse with experience in community nursing.
  • Ability to work flexibly and adapt to changing clinical needs.
  • Strong communication, interpersonal and organisational skills.

Additional Information

Service operates from 8 am to 8 pm, seven days a week. An evening and night nursing team supports flexible coverage.
