A healthcare provider in Dudley seeks candidates for a role focusing on enhanced health care in care homes. Responsibilities include care assessment, treatment planning, and working within a multidisciplinary team. The ideal candidate will be an independent prescriber with experience in healthcare settings. This role supports a new service aiming to improve health outcomes in the care home sector.
This is a new exciting opportunity for the right individuals to help establish a brand new service and support the provision of the Enhanced Health in Care Home aspect of the National DES PCN Contract. The hope is this service will reach across large proportions of the Care Home Sector within Dudley giving a first class provision of health care to those residents who fall under the scope of this new service.
The vision for this service is that it will be regionally leading and improve the health care offering to the care home population enabling a more proactive health care vision with prevention and admission avoidance at the heart of the delivery model.
The post holder will either have Non-Medical independent or supplementaryprescriber status. The post holders will work in conjunction with the appropriate multidisciplinary teams(MDT) across Dudley.
The aim of the Enhanced Care Home Team will be to support the provision for the "Enhanced Health in Care Home DES" working in a small team of MDT health care professionals to provide this provision across approx. 40 care homes within the Dudley Borough.
The right candidate will act autonomously within their professional scope of their clinical remit, providing care for patients/residents through face-to-face assessments, telephone consultations or care home visits via the weekly ward rounds.

Delivery of care in line with NICE guidelines and evidence-based care. Identify patients who are entering the final months of life utilising the Gold Standard Framework (GSF) pro active identification guidance. Have an awareness and understanding of EOL stages/care including the need for advanced care planning or ReSPECT documentation implementation. Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate. Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved. Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice. Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan. Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc. Assess, diagnose and treat patients/residents within care homes who require medical attention and/or refer to appropriate agency if/as required. Refers patients to practice teams, secondary care organisations and other organisations as necessary. Responsible for associated admin aligned to the role such as EMIS updates.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
