A mental health service provider in Plymouth is seeking a professional to coordinate care for individuals with complex mental health and substance misuse needs. Responsibilities include conducting comprehensive healthcare assessments and managing treatment packages. The ideal candidate will have experience in mental health care and proficiency in electronic case management systems. To work as part of the Complex Needs Team within the Harbour Partnership, as part of an integrated, multi-disciplinary/multi agency team, co-ordinating the care for individuals who have a complex range of mental health and substance misuse needs and other complex comorbidities. To develop the interface between the Harbour Partnership and other services and agencies within the Plymouth Alliance so that people who use services and their carers are offered focused and co-ordinated packages of care which are appropriate to their needs. To participate in line management, appraisal, supervision, teaching, recruitment and day-to-day management tasks in relation to your role within team. To provide and coordinate complex packages of treatment which may include the administration of medicines.
This post is a key part of the mental health offer within the system and although, based within Harbour the post holder will be responsible for managing clinics across the city in a range of settings. To provide a comprehensive healthcare assessment including risk assessment and be responsible for providing and coordinating a recovery orientated package of care alongside other key services and professional stakeholders. To use both Halo and System one electronic case management systems and to fulfill obligations in relation to NDTMS and Treatment Outcome Profile. (TOP).
To provide medicines management and psycho education to a group of people receiving Opiate substitute therapy alongside other medicines for a range of mental health and physical health conditions.
