A leading healthcare provider in London seeks a Medicines Management Pharmacy Technician to deliver pharmacy services in accordance with legislation and clinical requirements. You will work with pharmacy and clinical teams to manage medications for patients, participate in discharge planning, and ensure safe medication practices. This role offers a competitive salary from £39,000 to £45,000 based on experience.
Reporting to the Lead Medicines Management Pharmacy Technician, the Medicines Management Pharmacy Technician will provide an exemplary pharmacy service in accordance with current legislation, accepted professional and ethical standards and clinical requirements. The post-holder will focus on medicines management for outpatient and inpatient service developments. You will work alongside the Clinical Pharmacists and other clinical teams to develop and implement the current medicines management strategies, as well as day to day support of these areas upon opening of the hospital.
Prioritize and obtain accurate medication histories as part of the pre-admission process.
Identify newly admitted patients to obtain/confirm an accurate medication history or to highlight to the ward pharmacist any existing patients who have had their prescribed medicines changed (added/stopped/dose adjustments/route change).
Facilitate in a timely fashion the supply of any non-stock medication for individual patients.
Attend regular discharge planning meetings to gather information on potential discharges.

Raise any relevant issues with the multi-disciplinary team to ensure resolution prior to discharge
Assess that any Patients Own Medications brought in from home are suitable for use and to facilitate safe use of medicine upon discharge
Re-label any Patients Own Medication that has had a dose change according to local policy
Counsel patients, their carers, and healthcare professional on doses, frequency, side effects, benefits of taking medicines/completing courses of medication, interactions with other medicines and using relevant resources to facilitate patient education
Signpost patients to other healthcare professionals who may be able to assist with advice or compliance issues when the patient returns home
Undertake the final accuracy check for dispensed medicines.
Label and dispense prescriptions from outpatients, wards and other clinical areas, ensuring that standard operating procedures are followed
Ensure effective stock management at the clinical areas to meet the clinical services needs and patients treatment plan
Receive, document, store and dispose of Controlled Drugs with Pharmacist or authorised witnesses as per local policy
Support the dispensary team where needed
Adhere to relevant Finance related policies including authorisations and billing
Work with relevant pharmacy Caregivers in managing medicines stock
