Job description
Overview

The post holder will be the point of contact for all administrative issues relating to patient referrals and appointment bookings. The post holder will work as part of a multi-disciplinary team and will provide support to the Senior Sister or Medical Day Unit Nurse-In-Charge in proactive management and screening of incoming referrals, booking appointments, sourcing clinical documentation and diagnostic results from internal and external care providers, communicating with members of the multi-disciplinary team and patients, and ensuring pathways are managed smoothly.

Responsibilities
  • Act as a customer-focused, accessible point of contact to ensure high levels of patient and referrer satisfaction and to provide a fast, efficient, and effective service in accordance with trust and ward policy.
  • Liaise directly with patients and the referring team; manage incoming referrals, book appointments, and obtain clinical documentation and diagnostic results from internal and external care providers.
  • Communicate professionally with a range of professionals within the Medical Day Unit team and support proactive management of patient pathways.
Collaboration and Communication
  • Work as part of a multi-disciplinary team and maintain effective communication with patients, families, and care teams.
  • Demonstrate good IT skills and an excellent telephone manner as vital components of the role.
