Description:

Part or Full time LOCUM DENTIST required for 2-5 days (FLEXIBLE) in a busy, mixed, well established INDEPENDENTLY OWNED practice near Keelham, West Yorkshire.

• £400 PER DAY - for 30 UDAs + £11 per UDA for any over
• 50% split on private
• 50% split on lab bills
• Fully computerised 2 surgery practice with SOE Software
• Digital x-rays & Rotary Endo
• Existing well maintained patient list
• Full support team with Hygienist
• 15 minutes to Bradford & 45 minutes to Leeds
• Available now

Compliance requirements:

• Full NHS Performer Number
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
• IRMER
• PIN number
