Overview

We have a vacancy for a part time Band 6 learning disability nurse in our health facilitation team. The main function is to work closely with adults with learning disabilities to ensure they get their health needs met. The role supports people with a variety of health issues, including physical health and access to primary or secondary care, emotional wellbeing or mental health concerns.

The post holder will work collaboratively with people with learning disabilities, their carers, professionals and agencies to provide a high quality, comprehensive service to assess and plan health care, with the central aim of maintaining and improving the health and wellbeing of adults with learning disabilities. This will include direct work with service users who have complex healthcare needs, care coordination, supporting others to act as Health Facilitators, supporting health providers to make reasonable adjustments to care pathways and engaging in a range of health promotion activities.

Responsibilities
  • Support adults with learning disabilities to have their health needs met, including physical health, primary/secondary care access, and mental health or emotional wellbeing concerns.
  • Work independently to support individuals, while liaising closely with other health colleagues within the MDT and with primary or secondary care, social care staff, education, paid providers and family carers; excellent communication skills are required.
  • Travel and conduct home visits across the Bradford district; role based in the health facilitation team at the main office in Waddiloves, Bradford.
  • Collaborate with people with learning disabilities, their carers, professionals and agencies to assess and plan health care and promote health and wellbeing.
  • Work with service users who have complex health care needs, support care coordination, and assist others to act as Health Facilitators.
  • Support health providers to make reasonable adjustments to care pathways and engage in a range of health promotion activities.
Requirements and Skills
