Overview

The shift pattern for this post is: Monday - Friday

An exciting opportunity has arisen for a lead patient flow facilitator within the medical specialities of ECMS RAH.

The post holder will support all aspects of patient discharge planning and support the facilitation of discharge and flow through the medical wards. As lead you will support the patient flow team, providing leadership and guidance. You will work alongside various members of the senior management team daily to ensure service delivery. The post will be worked over Monday – Friday pattern.

Responsibilities
  • Lead the patient flow team and provide leadership and guidance in discharge planning and flow through medical wards.
  • Support all aspects of discharge planning and facilitate patient flow to improve service delivery.
  • Collaborate with the senior management team to ensure effective service delivery on a Monday to Friday pattern.
