A health service provider in Scotland seeks a Discharge Co-ordinator for a 6-month contract. This role involves managing discharge processes from Forth Valley Royal Hospital, ensuring safe and timely transfers for patients. Ideal candidates must be a registered nurse or AHP. Weekend work on a rota basis is required. Strong organisational and communication skills are essential for success in this role.
Secondment/Fixed term opportunity for 6 months to improve service delivery.
We currently have an exciting opportunity to join the Discharge Team on a fixed term basis for six months. Working as a Discharge Co-ordinator you will, as part of a wider team, manage the daily process of pro-active Discharge Planning within Forth Valley Royal Hospital to support the safe and timely transfer / discharge of patients from the Acute Hospital, while implementing the DWD (Discharge without Delay) principles and a Home First ethos.
The post may be available as a secondment. Existing permanent employees must discuss this opportunity with their substantive line manager and complete the secondment request form prior to interview. The secondment policy and form are available on the HR/Recruitment intranet pages and must be completed and signed by the substantive line manager. The candidate should take it to interview as confirmation that an agreement has been granted.
