Overview

Secondment/Fixed term opportunity for 6 months to improve service delivery.

We currently have an exciting opportunity to join the Discharge Team on a fixed term basis for six months. Working as a Discharge Co-ordinator you will, as part of a wider team, manage the daily process of pro-active Discharge Planning within Forth Valley Royal Hospital to support the safe and timely transfer / discharge of patients from the Acute Hospital, while implementing the DWD (Discharge without Delay) principles and a Home First ethos.

The post may be available as a secondment. Existing permanent employees must discuss this opportunity with their substantive line manager and complete the secondment request form prior to interview. The secondment policy and form are available on the HR/Recruitment intranet pages and must be completed and signed by the substantive line manager. The candidate should take it to interview as confirmation that an agreement has been granted.

Responsibilities
  • Manage the daily process of pro-active Discharge Planning to support safe and timely discharge from the Acute Hospital.
  • Engage with patients, carers, hospital colleagues and Health and Social Care teams to ensure robust discharge plans.
  • Work within the Discharge without Delay (DWD) principles and the Home First ethos.
  • Prioritise fluctuating workloads, maintain focus across professional boundaries, and operate in a busy environment.
  • Demonstrate good organisational skills, effective communication, problem solving, and commitment to continuing professional development.
Qualifications and Requirements
  • This post is open to 1st Level Registered Nurses and AHP registered with HCPC.
  • The service covers 7 days a week, 8am-4pm; weekend work on a rota basis is part of the role.
  • The post may be considered as a training position under Annex 21 of the Agenda for Change terms & conditions.
  • PVG scheme membership is required; more information is available on the PVG scheme.
