Job description

We have an exciting opportunity for an Integrated Care Nurse to join our Planned Care team at HMP Pentonville, a category B prison located in London.

HMP Pentonville is a male remand prison with a capacity of 1,200 inmates and is 10 minutes from Kings Cross station.

They have an on‑site staff canteen and gym, and the training and development opportunities are excellent.

Hours

37.5 hours a week, Monday‑Friday 7.30am‑4pm. Some flexibility required.

This is a full‑time position. The annual salary is up to £55,086 per annum, FTE‑based, depending on experience.

Main duties of the job

As an Integrated Care Nurse, your responsibilities will vary. You will:

  • Act as a Physical Health Nurse in mental health settings, bridging the gap between physical and mental healthcare.
  • Assess and manage the physical health needs of individuals with serious mental illness, often collaborating with multidisciplinary teams to deliver holistic, evidence‑based care.
  • Monitor vital signs, coordinate clinics, support staff training, and promote health awareness to reduce health inequalities and prevent diagnostic oversharing.
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
  • Use your skills and knowledge to deliver nurse‑led clinics to your patient group whilst being supported by a multi‑disciplinary team.
  • Provide outstanding evidence‑based care to our patients, seeking out, listening to and acting on their feedback so that care is personalised and informed by what matters to them.

Qualifications required
