Every day our Health Visitors provide high quality healthcare in people\'s homes or at convenient local clinics and children centres, helping children and families to stay well, manage their own health with the right support and avoid unnecessary visits or long stays in hospital. Lewisham and Greenwich NHS Trust (LGT) have exciting opportunities available for passionate and committed Health Visitors across the borough of Lewisham. We are looking for candidates with excellent clinical and communication skills to join our 0-5 yrs skill mix teams contributing to delivery of The Healthy Child Programme. Our latest CQC inspection awarded an outstanding rating for the Children\'s Community Directorate. The Health Visiting Service has been re-accredited by UNICEF UK as Baby Friendly Level 3 and we are working towards Gold standard accreditation. MECSH training and supervision operate with regular opportunities to become a MECSH trainer. Term Time Hours Available.
Responsibilities
- Provide high quality health visiting services to a defined population, employing the principles of health visiting.
- Maintain and improve health and life chances for children and families through health needs assessment, health promotion and participation in public health strategies.
- Deliver planned and opportunistic services in home, child health clinics and other settings, working as part of the primary health care team and in collaboration with statutory and voluntary agencies.
- Manage an individual caseload and delivery of health visiting services in line with local delivery plans and government targets.
- Deliver a planned programme of health promotion for children under five in partnership with GP practices and children\'s centres; identify local health needs and develop strategies to meet them.
- Provide a core health visiting service to all families in the defined population and assess, develop and implement individualised care plans with parents/carers.
- Assist in breastfeeding promotion, identify and support those at risk of postnatal depression, encourage smoking cessation and support families with parenting using advanced communication and counselling skills.
- Work with mental health needs, participate in psychological assessment reviews and use appropriate tools including EPDS, PSQ and GAD where applicable.
- Attend and contribute to child protection processes, including case conferences, care reviews and strategy meetings when needed.
- Maintain accurate, comprehensive records and provide data for audits and reporting as required by the Trust.
- Provide peer support within a primary care setting for vulnerable families and children protection cases.
- Engage in clinical supervision and facilitate professional meetings for the skill-mixed health visiting team.
- Identify local health needs and develop strategies to meet them; work with GPs, social care and other service providers to improve outcomes.
Qualifications & Requirements
- Adhere to the NMC Code of Professional Conduct and be conversant with the Scope of Professional Practice and related advisory papers; maintain clinical competence through continuing professional development.
- Demonstrate evidence-based practice within a clinical governance framework and participate in audit/research to benchmark and improve practice.
- Maintain policies and procedures, including those related to complaints, and keep accurate records; contribute to policy development as required.
- Be a nurse prescriber and attend regular updates where applicable.
- Promote and support breastfeeding; support families in parenting and parenting programmes using effective communication and counselling skills.
- Support clients with mental health issues and participate in relevant assessments and reviews.
- Be able to work within the Trust\'s Child Protection Policies and Procedures and engage with social care and health partners as required.
