Working within the Service Delivery Team as a Health Coach within the Weight Management Interface Hub, you will support adults and families who are overweight or obese to achieve sustainable lifestyle change through person‑centred, weight management programmes. Drawing on your experience of helping individuals with lifestyle modification, you will deliver nutrition, physical activity and behaviour change interventions through group, one‑to‑one, community and digital formats. You will build strong relationships with diverse communities across Leeds, monitor service user progress using SMART goals, and work collaboratively with partners to reduce health inequalities and improve long‑term health outcomes
Working as a Health Coach you will be working with adults and/or families to support, motivate and advise those who are currently overweight or obese.

ROLE SUMMARY

MAIN DUTIES
Working as a Health Coach you will beworking with adults and/or families to support, motivate and advise those whoare currently overweight or obese.
You will be passionate about workingwith individuals to begin the life‑changing processes necessary for permanentand continued weight loss and weight maintenance.
You will understand challenges andbarriers that adults face with relation to their health (both physical andmental) and wellbeing. You will engage with and build a good rapport withadults from varying communities across Leeds.
You will deliver our programmes ofweight management as well as supporting the development of new nutrition andexercise programmes that meet the needs of our service users.
The ideal candidate will be passionateabout promoting healthy lifestyles and delivering motivational change which is personcentered.
You will have a background in healthcoaching, nutrition, exercise, public health, community health or adult and/orfamily weight management and be an excellent team player and have a willingnessto learn and develop.
Duties andResponsibilities will include;
ServiceDelivery
Being part of a wider team that will deliverour adult weight management programme including nutrition, behaviour change andexercise delivery to groups and though one‑to‑ones, in the community andthrough digital methods such as video and telephone consultations.
Deliver evidence‑based educationaround nutrition, physical activity and behavioural change
Deliver programmes with a person centeredapproach using skills of positive psychology and motivational interviewing
Monitor service user progress throughregular consultation and identifying specific goals for success
Working with individuals and groupsusing SMART goal setting
Keep detailed consultation notes,recording data accurately, ensuring all forms and other documentation iscompleted correctly
Be able to organise own workload inorder of priority and deadlines, daily administration tasks and otherwork‑related activities.
Undertake and complete all mandatoryand additional skills training as required
Service Development
Work with service users and internal/externalstakeholders to co‑produce meaningful session materials
Contribute to the contract performanceby submitting regular case studies to the line manager
Contribute to training and support newstaff
Build community links and partnershipworking to encourage new referrals into the service and maintain close workingrelationships with other community services.
Relationships
Present service and referral pathwayinformation at stakeholder meetings/community events as directed by the ServiceDelivery and Relationship Manager
To attend and actively participate inteam meetings, such as MDT

To demonstrate personal andprofessional development in line with ethos of lifelong learning
To maintain statutory and mandatorytraining in line with the Leeds GPConfederation training matrix

To work within the Leeds GPConfederation framework of clinical governance in order to maintain and improvepatient care, quality and efficiency
To succeed inthe role, the successful candidate must have;
Significant experienceof supporting people make positive lifestyle change
Significant experiencein working with service users with needs arising from being overweight andobesity
Significant experiencein working with members of the public from varying backgrounds
A passion for helpingto reduce health inequalities and supporting disadvantaged groups
Working to tightcontractual deadlines and controls
