Overview

We are looking for an ambitious, dynamic, motivated, compassionate and enthusiastic Band 5 nurse to join our Community Nursing team in Beds. The post is to join Mid Beds Community Nursing Team, who deliver responsive high quality nursing to patients. We are passionate about patient-centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs. The Service operates between 08.00 to 21.00 hours and successful candidates will work a variety of shifts, including weekends, evenings and bank holidays. Car driver and access to a car in order to carry out service duties is essential. Post is permanent / fulltime.

The post holder will be required to travel throughout the service area according to the needs of the service e.g. Bedfordshire.

Responsibilities
  • Work as an autonomous practitioner to provide nursing care to patients in their own home.
  • The post holder will be responsible for the planning, implementation, and evaluation of individualised nursing care in a range of settings.
  • The post holder will provide health education to enable patients to self-manage, and achieve optimum independence following deterioration or a period of ill health.
  • The post holder is a member of the integrated locality nursing team where this is in existence.
  • To participate in the training of students, the induction of new staff and others that have contact with our services.
  • The post holder will be required to support the senior nursing team in the maintenance of efficient and effective delivery of services.
  • The post holder may be required to take responsibility for the team and caseload in the absence of senior staff.
  • Use a broad range of nursing knowledge, interventions and skills, to prevent, where possible and avoid hospital admission.
  • Develop good communication links with patients and carers and joint working with other services across professional and organisational boundaries in order to provide a patient-centred, holistic and seamless service.
