Job summary

A healthcare organization is seeking a Community Matron in Minehead to provide advanced clinical assessment and care for adults with complex, long-term conditions. The ideal candidate will be a registered nurse with experience in community nursing and knowledge of NHS policies. Community Matron - Minehead Medical Centre

Location: Community-based – Minehead Medical Centre & patient homes

Hours: Part‑time 20 hours available (Full‑time 40 hours)

Key Responsibilities
Clinical Care
  • Provide advanced clinical assessment, diagnosis and management of adults with complex, long‑term conditions — including frail, elderly and housebound patients — in their homes, care homes and community healthcare settings.
  • Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
  • Work closely with families, carers and social care teams to support older patients with complex needs.
Leadership & Coordination
  • Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations.
  • Support the development of proactive frailty identification and early intervention within the practice and PCN.

About you – ideal candidate

You will be someone who enjoys building strong, trusting relationships with patients and their families. Given the needs of our local population, experience in supporting older adults – including frailty, geriatrics, care homes or complex long‑term conditions – is highly valued.

Essential Qualifications and Experience
  • Registered Nurse with valid NMC registration
  • Independent Nurse Prescriber (V300)
  • Significant post‑registration experience in community or primary care, managing complex caseloads
  • Experience of multi‑agency, multidisciplinary working
  • Knowledge of current NHS policy, community nursing models, and primary care priorities
