Sirona's Community Homeless Health Team supports individuals who are homeless, sofa surfing and in temporary and emergency accommodation. We help 'bridge the gap' and promote re-engagement with health care.
This is an opportunity to engage with individuals facing complex health challenges, including those experiencing homelessness. You'll deliver holistic, evidence-based care -- from assessment and diagnosis to treatment and follow-up -- while working both independently and as part of a strong multi-agency team.
