Overview

Opportunities have arisen for a Community Assessment Officer within Salford's Adult Safeguarding Team. We are looking to recruit workers to help us deliver our community led support vision. You will be the first point of contact for the person requesting support and will undertake strength-based conversations to maximise wellbeing with a person-centered, compassionate approach. You will work closely with other organisations to help people access statutory or voluntary services within their local community.

The Adult Social Care workforce aims to improve outcomes for adults in Salford by empowering individuals, adopting a strength-based approach, promoting independence, and working in partnership with residents so their experiences, opportunities, and lives are as good as they can be. This includes maintaining a high standard of work when carrying out statutory functions on behalf of Salford Adult Social Care, including assessing people under the Care Act (2014) and connecting them with local communities to promote well-being. With appropriate support you will undertake assessments of a person’s mental capacity, considering the specific decision that needs to be made. You will complete support planning with individuals and their carers/families, undertake reviews to ensure needs are met and well-being is promoted, and use the social care system to record documentation such as assessments, support plans, and reviews.

Home visits and social care assessments may include assessing carers’ responsibilities for children, and participation in MDT meetings with health, education or other professionals.
