A regional healthcare provider is looking for experienced community nurses to join their Clinical Decision Making team. Your role will involve triaging referrals, providing clinical advice to nursing teams, and conducting assessments over the phone. Our Clinical Decision Making (CDM) team is based within our Single Point of Access (SPoA) for Community Services in Wye Valley NHS Trust. The team is growing to support the development of our community services, enhancing our offer of urgent care and admission avoidance support for community nursing teams. We seek enthusiastic and experienced nurses with a robust understanding of community nursing who can provide clinical advice and support.
