Position Overview

Our Clinical Decision Making (CDM) team is based within our Single Point of Access (SPoA) for Community Services in Wye Valley NHS Trust. The team is growing to support the development of our community services, enhancing our offer of urgent care and admission avoidance support for community nursing teams. We seek enthusiastic and experienced nurses with a robust understanding of community nursing who can provide clinical advice and support.

Key Responsibilities
  • Clinically triage direct telephone referrals and determine, in collaboration with the patient, carer, or professional, the most appropriate service to avoid a crisis and/or admission.
  • Provide clinical advice and support to community nursing teams.
  • Conduct remote assessments over the phone as required.
  • Coordinate support for patients who need urgent or same‑day service.
  • Support the Community Referral Hub, which manages referrals and patient queries for District Nurses, Urgent Response Service, Virtual Wards, Virtual GP, IVOPAT, Therapies (OT/PT), and Falls Support.
  • Collaborate with Administrators and Call Handlers to ensure efficient and effective service delivery within the hub.
  • Assist junior staff in gathering information and maintaining the seamless day‑to‑day running of the hub in support of clinical leads and administration managers.
