Job Responsibilities
  • Provide visible leadership and line management for the Integrated Care Team (ICT), supporting other teams when required.
  • Offer clinical expertise and advice in the management of complex cases, palliative care, and safeguarding, ensuring high standards of care at home.
  • Ensure all team members are clinically competent, well-supported, and working within professional guidelines.
  • Maintain oversight of record-keeping standards, including medication charts and documentation audits, ensuring compliance with NMC and organisational policies.
  • Work closely with the Hospital Discharge Team and wider system partners to support safe, timely discharges and reduce avoidable hospital admissions.
  • Promote and embed clinical supervision, staff development, and evidence-based practice across the team.
  • Support and lead service development, using audits and performance data to drive improvements and meet local and national targets.
  • Manage complaints, incidents, risks, and staff concerns, escalating appropriately and ensuring lessons are learned.

We are seeking an experienced and motivated Registered Nurse (RGN) with:

  • A relevant degree (or equivalent experience), plus teaching/assessing qualifications.
  • Significant post-registration experience, including at Band 6 level or equivalent, ideally in a community setting.
