A healthcare provider in Nottingham seeks a passionate Clinical Pharmacist to join their multidisciplinary team. This role focuses on optimizing medication use and supporting patient care within a general practice setting. You will provide expertise in clinical medicines management, engage with patients and healthcare professionals, and be instrumental in enhancing quality and efficiency in healthcare services. The successful candidate will possess an undergraduate degree in pharmacy and relevant prescribing qualifications, with at least 2 years of experience in primary care. Village HealthGroup is a thriving busy dispensing practice serving 27,000 patients. Basedover 4 sites we have a large multi-disciplinary team led by our GP Partnerswhich comprises GPs, Nurses & HCAs as well as Clinical Pharmacists, NAs, tNAs & Social Prescribers as part of our wider PCN ARRS team. Our clinical team is supported by our administrative, secretarial & reception teams.
We have anexciting opportunity for a Clinical Pharmacist to join our practice team workingalongside our PCN pharmacy team. You will be a prescriber or in the process ofcompleting this and will work as part of the general practice team within theirclinical competencies to provide expertise in clinical medicines managementacross a range of aspects such as day to day medication related queries, acuteprescription requests, supporting hospital discharge prescribing arrangementsand follow up, actioning of MHRA alerts and targeted medication reviews whichmay be done over the telephone or face to face. You will be expected to help instreamlining processes and developing protocols to allow for safer prescribingand to meet CQC expectations.
You will act asa specialist resource to answer medicines-related queries, to signpost to otherservices and to liaise with other healthcare professionals as required.Clinical supervision and mentorship will be provided by the GP Partners for thepharmacy team.
We are lookingfor candidates who are enthusiastic self-starters and who are adaptable tochange. You will have excellent communication skills and be proactive inapproaching patients, carers, colleagues and other agencies as and when needed.It is expected that you will be able to use your initiative to gatherinformation and to use the resources that are available to you, such as prescribing guidelines. Being a team player is essential. A good level ofcompetence in use of I.T, predominantly SystmOne is required.
The successfulcandidate will work within a supportive practice team and the wider PCNpharmacy team in order to deliver safe and effective use of medicines byapplying their clinical, pharmaceutical and technical skills to a variety oftasks within the medicines optimisation agenda.

THE PURPOSE OF THE ROLE
General Practice is at the heart of the Healthand Social Care system and it is critical to have thriving, sustainable generalpractices that meet local peoples health needs. The Clinical Pharmacist is instrumental insupporting the practice clinically. This is a key role, intended to support thedelivery and development of the pharmacist role in primary care and ensure that we continue to provide highquality, accessible and responsive services to both practices and patients.
KEY RESPONSIBILITIES
Areas of responsibility for all clinical pharmacists
Working with stakeholders
Recognise the relevant key multi-professional stakeholders across the IC to include, and not limited to, the wider health and social care system.
Identify and develop relationships with the practice team,private providers, the local primary care team, neighbouring PCNs and otherkey partners to enable good working relationships and promote and share bestpractice.
Develop networks with local,regional, and national clinical specialist pharmacy interest groups wherepossible.
Clinical Practice
Provide telephone, domiciliary and face to face medication reviews, and where competent long-term condition reviews and common/ minor/ self-limiting condition management. Priority areas being complex polypharmacy, frailty, asthma, COPDand people with learning disabilities.
Provide face to face and telephone medication expertise.
Be a prescriber or belooking to complete relevant training to become a prescriber.
Through holistic structured medication reviews, support patients with their medication to get the best from them, reduce waste and promote self-care.
Support with medicines reconciliation of hospital discharges and clinic letters.
Support the provision of patient care that helps tackle local health inequalities.
Critically appraise complex medicines information and then communicate this to patients, carers, and health care professionals. This includes supporting with medicinesinformation queries.
Review and support the repeat prescribing process to ensure patients are receiving their medication safely.
Maintain comprehensive documentation and records according to legal and local requirements. This includes recordingall relevant pharmaceutical information in the patients record andcommunicating in writing with other health care professionals.
Education and training
Provide training /teaching / education sessions to practice healthcare professionals andadministration staff including GPs, Nurses, Pharmacy Technicians andPharmacists.
Demonstrates self-development through continuous professionaldevelopment activity; working alongside senior clinical pharmacist toidentifying areas to develop.
Gives support to relevant clinical placements.
Research
Looksfor opportunities to contribute to relevant research in collaboration with a range of stakeholders toinnovate and improve patient-centred care.
Demonstrates ability to critically evaluate and review literature.
Demonstrates ability to identify where there is a gap in the evidencebase to support practice.
Demonstrates ability to generate evidence suitable for presentations at practice and local level eg through clinical audit.
Demonstrates ability to apply research evidence base into workingplace.
Leadership and management
Support the medicines optimisation agenda across the PCN toensure:
Clinical governance
Develop and co-ordinate the implementation of relevant clinical audits within the PCN to support improvements in care.
Participate in and promote the recording and discussion of significant learning events.
Support with the cascade of Adverse Drug Reactions (ADR) to the MHRA and ensure that any drug recalls /alerts are actioned within the PCN.

Care Quality Commission
To work with general practice team to ensure thatpractices are compliant with CQC standards where medicines are involved.
To be professionally and legally responsible andaccountable for all aspects of own work (including non-medical prescribing);maintaining own continuous personal development and professionalaccreditation as required by the General Pharmaceutical Council.
To work unsupervised and to use own initiative withinprofessional boundaries, for example to initiate, manage and record ownmeetings as necessary.
Plan and prioritise own workload to ensure an equitableservice across all practices and delivery of workplans within the PCN.
Confidentiality
To respect patient confidentiality at alltimes and not divulge patient information unless sanctioned by therequirements of the role.
Special Working Conditions
The post-holder is required to travel independently between practice sites (where applicable), and to attend meetings etc hosted by other agencies.
The post-holder may have contact with body fluids ie, wound exudates;urine etc while in clinical practice.
Health and Safety/Risk Management
Thepost-holder must comply at all times with the Practices Health and Safetypolicies, in particular by following agreed safe working procedures andreporting incidents using the organisations Incident Reporting System.
Thepost-holder will comply with the Data Protection Act (1984) and the Access toHealth Records Act (1990).
This role profilesummarises the main responsibilities and requirements of the role only. The post holder will carry out any otherduties as may reasonably be required by their line manager in line with thegrade / band of the post.
