We are looking for an experienced pharmacist who can shape the way we deliver primary care across our five sites. You will be part of an existing team of 3 pharmacists and pharmacy technicians; you will handle all pharmacy-related patient inquiries, as well as structured medication reviews, high-risk drug monitoring, medicines reconciliation, repeat medication facilitation, drug audits, and other clinical work depending on your competencies. As a Clinical Pharmacist, you will support the delivery of the new Network Contract DES Service specifications. This includes addressing over-medication, inappropriate antibiotic use, withdrawing medicines no longer needed through NHS-led programmes such as low-priority prescribing, and medicines optimisation more broadly.
