Job description
Overview

To be responsible for the caseload within the Children's Community Nursing Team, providing specialist community nursing care, advice, and support to children and their families within the home, school, or other locations.

Act as a resource within this specialist area across Health, Social Care, and Educational agencies, thereby promoting holistic family care.

Be responsible for the day-to-day management and delivery of care being carried out by the other members of the nursing team.

Be a key health professional responsible for meeting the health needs of identified pupils with complex chronic health conditions within schools, and other locations, planning and leading the implementation of individual care plans for the schools and other locations, liaising with fellow professionals, education staff, parents/carers, and other agencies as necessary.

Additionally, the children\'s community nursing team will offer a comprehensive palliative care package to children as needed, so the post holder will also be expected to participate in this 24-hour on-call service - for end-of-life care only.

This post involves participation in the rota for a Monday-Friday 8 am-6 pm service and a pattern of shift work, which may include covering 9-2 pm weekends and bank holidays if clinically needed.

Responsibilities
  • To adhere to the Nursing and Midwifery Council Code of Professional Conduct and to be conversant with the scope of Professional Conduct and other Nursing and Midwifery Council Advisory papers.
  • The post holder is expected to work within and practice according to all Bromley Healthcare\'s relevant policies, procedures, and guidelines, including the appropriate Code(s) of Conduct associated with this post.
  • To take responsibility for one\'s own professional and organisational knowledge in accordance with relevant statutory and local requirements, including revalidation.
  • To demonstrate active interest and lead the team in at least one key area of community children\'s nursing.
  • To work closely with staff from statutory organisations as well as parents and carers so that these adults can meet the daily needs of the patients/pupils to enable children to have access to education and other activities of daily living.
  • To ensure that the relevant Safeguarding Policies and Procedures are followed and that these are available to and understood by self, junior staff, and colleagues.
  • To participate in Bromley Healthcare\'s Clinical Supervision programme both for self and for other staff.
  • To act as a resource person for statutory and voluntary agencies with specific regard to the roles and responsibilities of the CCNT.
