Job description

We are looking for an outgoing and enthusiastic Care Navigator to join our Reception and Administration Team on a full-time basis. We are a friendly, busy practice with a great team who strive for excellence in patient care and staff wellbeing.

Responsibilities

• Management of patient enquiries via telephone and in person
• Taking messages and booking appointments in surgery
• Arranging home visits
• Administering the waiting room and appropriate systems
• Making appointments for in-house clinics and attached services
• Chaperoning when required
• Liasing with GPs & practice staff
• Carrying out tasks
• Dealing with post & emails
• Document and Records Management: Registration of new patients, scanning and read coding of incoming mail onto patient electronic notes, management of repeat prescriptions, support medicines management where required, prepare prescription or query, forward to GPs, liaise with GPs and Reception/Administrative Team

This is a busy inner-city practice of 9,900 patients. The practice has 3 Partners, 9 salaried GPs, a Pharmacy team, Nursing team as well as a dynamic Administration team. The Elms is a training practice for GP registrars.
