A local healthcare practice in Ledbury seeks an Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to provide high-quality planned care in care homes. Responsibilities include conducting ward rounds, managing patient care plans, and working independently alongside GPs. The ideal candidate is organized, an efficient communicator, and has experience with older adults. The role is part-time, requiring three days of work per week.
The role's main focus will beto provide high quality Planned and proactive care for care homepatients in a weekly ward round; including but not limited to:
- supporting the practice with ongoing managementof care home patients by reviewing patients and amending Personalised Care& Support Plans and RESPECT forms.
- supporting care home patients withanticipatory care discussions and planning.
- providing acute medical treatment for care homepatients with undifferentiated and undiagnosed problems
- assessing and managing patients withundifferentiated and undiagnosed health care needs including investigating, treating,and referring patients as appropriate.
- Independently prescribing (where applicable to therole) and review appropriate medications in accordance with local policies andevidence based medical practice and work collaboratively with the wider team tosupport patients and to promote medication adherence.
- ordering investigations, follow-up results andarranging appropriate management including seeking advice where appropriate.
- Actively take a personalised careapproach and population centred care approach to enable shared decision makingwith the presenting person.
- Supporting practices with ward roundsand MDTs.
CLINICALASSESSMENT:
To provide a first point of contact for carehome patients with undifferentiated and undiagnosed health needs at the mostappropriate time and place.
Provide comprehensive and supportive end of lifediscussions with palliative care home residents, ensuring a thorough advancedcare assessment and plan are developed.
Complete frailty assessments and provide plansusing this information.
Provide and receive complex information; develops communication skills/presentcomplex information to GPs where appropriate.
To provide advanced assessment skills in historytaking and clinical examination to formulate a diagnosis and initiate anappropriate plan of care, including referral to other health and social care professionals,as necessary.
Instigate and interpret diagnosticinvestigations as appropriate.

5 Partner GP Team supported by multidisciplinary clinicalteam of Salaried GPs, Nurses, HCAs, Paramedic Practitioner, ANP, P, Pharmacists and Pharmacy Technician, First Contact Practitioner and Care Co-ordinators.

We are looking for an ANP or ACP to work 3 days per week carrying out ward rounds to our care homes. You will work together with the GP who has responsibility for the patients residing in the care homes, and you will have the expert help of a dedicated care home coordinator who will manage the administrate and planning side of your rounds.
The ideal candidate will be an excellent communicator, organised and efficient and able to work independently.
