A healthcare provider in Calderdale is seeking a Care Coordinator to support the delivery of personalized care services. Our goal is to provide excellent services and outcomes for the patients and communities by working more closely across local health, social and community care organisations.
Our Care Coordinators play a crucial part in achieving this and are integral the PCNs multidisciplinary team, working alongside Social Prescribing Link Workers, Physiotherapists and our Pharmacists to provide an all-encompassing approach to personalised care services., Undertake a variety of administrative duties to assist in the smooth running of the PCN, including provision of clerical support and information processing.
Support the coordination of care in care homes, including the delivery of care home rounds.
Act as the first point of call to patients and visitors to the Lower Valley Primary Care (PCN) building, projecting a positive, welcoming image to patients and visitors, either in person or via the telephone.
Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
Hours: 37.5hrs, Monday Friday, 8am 4pm
