Job Title: Care Coordinator

Salary: £27,000 per annum + on-call bonus

We are currently seeking a highly organized and compassionate Care Coordinator to join our busy domiciliary care agency based in Wymondham. The ideal candidate will have excellent communication and problem-solving skills, as well as a strong understanding of the care industry. This is a full-time position with the potential for growth within the company.

Responsibilities
  • Coordinating and managing the care schedules for clients, ensuring all visits are covered and caregivers are allocated appropriately
  • Liaising with clients, caregivers, and other healthcare professionals to ensure the highest quality of care is provided
  • Conducting regular assessments and reviews of clients' care needs and updating care plans accordingly
  • Participating in the recruitment and training of caregivers to maintain a strong and reliable team
  • Managing and maintaining accurate records and documentation in line with company policies and procedures
  • On-call responsibilities, including responding to emergencies and providing support to caregivers and clients outside of office hours
