Overview

An exciting opportunity for a suitably qualified and experienced senior mental health practitioner to work closely with the Team Manager and other senior clinicians within H&F Central MINT services in Hammersmith & Fulham. This post is open to nurses, social workers and occupational therapists and is a management post within the team. The remodelled community teams deliver mental health interventions in partnership with primary care colleagues and continue to provide care for those with more complex needs.

The Long-Term Plan (LTP) and NHSE’s Community Mental Health Framework provide a historic opportunity to address gaps in community mental health care. Community teams are moving towards joined-up care and whole population management to support adults and older adults with severe mental illnesses.

Responsibilities
  • Lead and monitor the performance and care delivery of the nursing staff in the MINT team, providing local professional leadership.
  • Participate in the review of clinical policies and guidelines relevant to the area and support and implement changes to practice.
  • Liaise with other organisations and professionals to promote communication and good practice.
  • Undertake audit activity and provide supervision to other staff in the service area/care group.
  • Support the assessment of care needs and the development, implementation and evaluation of programmes of care for a small, defined caseload and maintain related records.
  • Provide effective leadership within a clinical team to deliver a high-quality service within available resources and defined geographical area.
  • Demonstrate an understanding of psychosocial interventions, including psycho-education, wellness recovery action plans and discharge planning.
