A healthcare provider in London is seeking a Senior Mental Health Practitioner to oversee care delivery in the MINT team. This management role requires experience in leading teams and providing high-quality mental health interventions. The ideal candidate will have a background in nursing, social work, or occupational therapy, focused on supporting adults and older adults with severe mental illnesses. This position offers an opportunity to impact community mental health services positively.
An exciting opportunity for a suitably qualified and experienced senior mental health practitioner to work closely with the Team Manager and other senior clinicians within H&F Central MINT services in Hammersmith & Fulham. This post is open to nurses, social workers and occupational therapists and is a management post within the team. The remodelled community teams deliver mental health interventions in partnership with primary care colleagues and continue to provide care for those with more complex needs.
The Long-Term Plan (LTP) and NHSE’s Community Mental Health Framework provide a historic opportunity to address gaps in community mental health care. Community teams are moving towards joined-up care and whole population management to support adults and older adults with severe mental illnesses.
