Overview

Exciting opportunity for a Band 6 Senior Locality Hub Clinician. The role involves autonomous telephone triage and clinical assessment, supporting a system‑wide approach, prioritising care in the community, and preventing unnecessary hospital admissions.

The post is a locality role covering different community nursing teams on a rota basis. The Clinical Hub Coordinator, under the supervision of the Clinical Team Leader, manages the coordination of patient care within the locality by triaging referrals and messages to ensure patients are on the correct clinical pathway and undertakes clinical hub functions, including in‑reach into the aligned acute hospital. The post holder will be accountable for the delivery of safe, high‑quality care within the locality across various settings.

Responsibilities
  • Provide a comprehensive telephone triage and clinical assessment service to patients and/or their carers/relatives, ensuring safe and appropriate clinical decisions are made.
  • Triage referrals and messages to determine when and if a patient requires a visit, establishing the correct clinical pathway based on the service and its priority.
  • Demonstrate critical thinking and assessment skills when making clinical decisions, including handling complex cases that may require an MDT approach.
  • Act as an autonomous practitioner dealing with emotional and sensitive situations, respecting Trust values.
  • Undertake the locality clinical in‑reach function, collaborating with the aligned acute hospital to identify patients suitable for discharge with community support.
  • Support the monitoring and triage of alerts for patients in local care homes via electronic remote monitoring, initiating the correct clinical pathway and follow‑up care.
  • Utilise clinical systems and gather relevant information to support decision making, requiring prolonged concentration.
  • Obtain appropriate input and direction from internal and partnership clinicians and professionals to determine the best clinical course of action.
  • Participate in other locality hub functions to ensure efficient, effective and timely responses, reducing length of stay, preventing admissions, and improving patient safety.
  • Provide advice and support to other clinicians in the delivery of patient care.
  • Ensure fair treatment of patients, carers, and colleagues in line with Trust Equality, Diversity and Inclusion policies.
  • Report and act on any incidents involving potential or actual abuse, protecting individuals and informing the senior clinician on duty.
  • Maintain accurate clinical records and documentation in accordance with the NMC Code of Conduct.
  • Deputise in the absence of the Team Leader to coordinate day‑to‑day management of locality hub functions.
  • Engage proactively with service users, encouraging feedback and acting on evidence from user responses.
  • Undertake other duties commensurate with this grade in agreement with the line manager.
