Responsibilities
  • Work as a Care Coordinator within a multi-disciplinary team to assess, plan, and review health and social care needs under the Care Programme Approach (CPA) for service users with a forensic/offending background.
  • Undertake comprehensive assessments alone or with colleagues, providing feedback to the Multi-disciplinary Team to ensure individualised and appropriate patient care.
  • Deliver effective case management for a defined caseload in line with the Care Programme Approach, collaborating closely with various departments and external health, social care, and forensic services.
  • Ensure effective discharge care planning to support patient transitions.
