Overview

Would you be interested in a career within the Community as a Band 4 Health care Assistant, in the Mid Beds Community nursing team? We are looking for two Band 4 to join our nursing team with the motivation to progress to an NA. You will be contributing to the development and delivery of high quality evidence based patient care to an identified group of patients.

We are looking for two dynamic, motivated and enthusiastic people who have excellent communication, IT literate and problem solving skills alongside a passion for patient centered care. You will work corporately within our Primary Care Team and will be supporting the Community Nurses and the wider MDT to provide high quality care to housebound residents who live with a wide range of physical and mental health conditions. You will be a team player with a professional approach to care and act as a good role model for other members of the team.

Nursing Associates are encouraged to apply to support the delivery of quality community care and be a key part of the future model of community care. You will be involved in urgent community response and virtual ward as well as planned care to a caseload of people.

There are two posts available based in Shefford Health Centre to deliver care to patients in Mid Beds and Steppingley Hub to deliver care to patients living in West Mid Beds.

Responsibilities
  • Contribute to the development and delivery of high quality, evidence-based patient care for an identified group of patients.
  • Support the Community Nurses and the wider multidisciplinary team (MDT) to provide high quality care to housebound residents with a wide range of physical and mental health conditions.
  • Work within the Primary Care Team and act as a professional role model; maintain a patient-centred approach.
  • Be involved in urgent community response and virtual ward activities as well as planned care to a caseload of people.
  • Be flexible and assist colleagues to cover service needs across the wider integrated team.
  • Access to a car is essential to carry out service duties.
Qualifications and Skills
  • Band 4 Health Care Assistant role with motivation to progress to a Nursing Associate (NA).
  • Excellent communication skills, IT-literate and strong problem-solving abilities.
  • A passion for patient-centred care and the ability to work effectively within a team.
Working Patterns and Support
  • Full induction programme provided.
  • The Community Nursing Service operates between 08:00 and 21:00 hours.
  • Rotational 7.5 hour shifts which will include weekend, evening and Bank Holiday working.
  • Two posts are based in Shefford Health Centre (Mid Beds) and Steppingley Hub (West Mid Beds).
