Overview

We have an exciting permanent opportunity for a Specialist Mental Health Practitioner to work in the Shore Medical Primary Care Network, comprising GP Practices based in east Dorset. The successful candidate will be an experienced Mental Health Practitioner with experience in a Primary Care and/or Community Mental Health Team setting.

This role is suitable for proactive, innovative professionals who are passionate about high-quality care for people identified as in need of mental health support in a primary care setting. There is a clear opportunity to develop your career and skill set in a creative environment.

Applications from candidates requiring current Healthcare worker sponsorship to work in the UK are ineligible.

Integrated Model and Service Focus

This position will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care. Services are focused around local communities and GP practices to help rebuild and reconnect healthcare teams. The practitioner will provide targeted care according to population needs and support the PCN clinical strategy. The practitioner will facilitate onward access to secondary care-based services, third sector services, and link with existing support services via the Primary Care Network.

Role Details

This post is full-time 37.5 hours, with consideration for job share or part-time working.

Employment requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants who subscribe to the DBS update service and present a valid DBS certificate may not require a new check.
