A healthcare provider in the UK is seeking a Specialist Mental Health Practitioner to join the Shore Medical Primary Care Network. This full-time role involves collaborating with GP practices to provide high-quality mental health support. The ideal candidate will have experience in Primary Care or Community Mental Health. We have an exciting permanent opportunity for a Specialist Mental Health Practitioner to work in the Shore Medical Primary Care Network, comprising GP Practices based in east Dorset. The successful candidate will be an experienced Mental Health Practitioner with experience in a Primary Care and/or Community Mental Health Team setting.

This position will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care. Services are focused around local communities and GP practices to help rebuild and reconnect healthcare teams. The practitioner will provide targeted care according to population needs and support the PCN clinical strategy. The practitioner will facilitate onward access to secondary care-based services, third sector services, and link with existing support services via the Primary Care Network.
This post is full-time 37.5 hours, with consideration for job share or part-time working.
