Advanced Social Work Practitioner – Hospital Team

Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Advanced Social Work Practitioner within the Hospital to work full time based in Hillingdon.
The salary for this permanent Advanced Social Work Practitioner job is up to £54,129 per annum.

  • To act as Duty Manager.
  • To mange daily discharges via bridging pathway and pathway 3 placements.
  • To triage cases alongside the Hillingdon HUB and out of borough hospital referrals.
  • Attending conferences calls with health professionals and being able to deliver results.
  • Supporting team members, allocating cases, management of stepdown beds.
  • Safeguarding Manager role, attending regular meetings with safeguarding lead at THH on progression of investigations.
  • Carry a small caseload of complex cases.
  • To support hospital discharges for our residents from Hillingdon Hospital as well as out of borough hospitals, rotating on a duty rota and to carry a small caseload of complex cases.
  • Committed to providing quality services that promote independence/wellbeing, control, choice and social inclusion. You will demonstrate a person-centred approach by keeping the service user at the centre of the assessment and discharge process.
  • Free parking (4 days a week) if based at the Civic Centre.
  • Generous holiday entitlement: a minimum 26 days a year (plus bank holidays), rising to 29 days after five years’ service. For more senior positions holiday entitlements can rise to a maximum of 37 days.

Requirements of this role:

  • Social work qualification with registration with Social Work England.
