Job description

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

The shift pattern for this post is 8am to 8pm, 5 over 7 including public holidays

Please be advised that, effective 1 April 2026, the full-time working hours for all Agenda for Change posts will be reduced to 36 hours

This is an exciting opportunity for enthusiastic and motivated advanced nurse practitioners to work within the developing Hospital at home service within Renfrewshire/East Renfrewshire. The hospital at home team will work across the Renfrewshire HSCP geographical area and parts of East Renfrewshire. The hospital at home model is aligned to the frailty pathway with the team working closely with front door teams at the RAH inclusive of ED, MAU, AMU and the Older Adults assessment unit.

The post holders will be based within the RAH and work alongside acute teams and flow navigation plus to admit patients with frailty presenting with acute changes to hospital at home virtual ward and provide acute level care within the person home or homely environment.

The service over the next few months is preparing to expand to increase the bed model to 18 beds delivered over 7 days 8am – 8pm inclusive of public holidays.

The post holders will require to use advanced assessment and decision making skills daily to complete initial assessment and diagnosis and work collaboratively with the responsible medical officer to agree care plan and treatment. This will include accessing hospital level diagnostic and completing necessary hospital level intervention at home. This is supported by SOP in place across the team to ensure safe and effective care delivery. The post holders will be key to the continuous assessment and decision making as part of the wider MDT of patients admitted to the virtual ward. They will lead the discharge planning of patients and will link with community services as required to ensure a safe discharge from hospital at home.
