The ideal candidate will work autonomously and collaboratively while offering flexibility in working hours. Key responsibilities include conducting assessments, prescribing medications, and managing patient care.
We are looking for an experienced Advanced Nurse Practitioner to join our friendly GP Practice. You will provide high-quality care, manage a broad range of clinical presentations, and work both independently and as part of our supportive team.
Salary: Negotiable depending on qualifications and experience. Days and hours of work are flexible.
The Advanced Nurse Practitioner will deliver patient care independently, within their professional scope of practice, through both in-person consultations and home visits.
The post holder will apply their expertise as a qualified Advanced Health Care Professional to independently manage patient caseloads, including treatment planning, referrals, admissions, and discharges, ensuring safe and effective care throughout.

Independently assess, diagnose, and manage patients with acute and chronic conditions
Conduct face-to-face consultations and home visits within scope of practice
Prescribe medications and refer patients to appropriate services when necessary
Manage patient caseloads, including treatment planning, referrals, admissions, and discharges
Perform clinical examinations and interpret diagnostic tests
Support long-term condition management and preventative care initiatives
Maintain accurate and timely clinical documentation in line with practice policies
Participate in clinical audits, quality improvement, and service development
Contribute to safeguarding and risk management processes
Collaborate with GPs, nurses, healthcare assistants, and administrative staff to ensure seamless patient care
