This is an exciting opportunity for an Advanced Clinical Practitioner to join and assist with the development of the Hospital at Home service, alongside virtual wards specialities within Older Persons Services.
The role will include assessments, diagnostics and implementing a treatment plan whereby patients conditions and plan of care can be met within their own home or within the community settings to prevent them from remaining within the acute settings when deemed appropriate.
Use of a car for transportation is essential.
We are looking for a dynamic, compassionate, organised and confident person who has experience in managing acutely unwell patients as there will be those situations when patients require acute assessment, care and escalation.
You will need to demonstrate the necessary skills to deliver services to patients with a wide range of complex clinical needs including frailty.
This role requires an active UK NMC Registration
Based Location: Christchurch Day Hospital but covering the whole of BCP area travelling
