What You Will Do:
1. Assessment and Planning

Carry out Care Act-compliant assessments and determine eligibility.

Co-create outcome-focused care and support plans with service users.

Conduct Mental Capacity Assessments and make Best Interest decisions when necessary.

2. Safeguarding

Lead or contribute to Section 42 enquiries.

Develop and manage safeguarding plans to address risk and vulnerability.

Apply statutory guidance and policy in all safeguarding duties.

3. Case Management

Manage a varied caseload including community support, hospital discharge, and long-term interventions.

Monitor and review care packages for quality and value.

Maintain accurate and up-to-date case notes.

Work alongside professionals in health, housing, and voluntary sectors.

Engage in multi-disciplinary meetings and shared decision-making.

Ensure joined-up, holistic support for adults and carers.

5. Professional Development

Maintain Social Work England registration and professional standards.

Take part in CPD, peer reflection, and development sessions.

Support service improvement through feedback and innovation.

Requirements:

Recognised Social Work qualification.

Current registration with Social Work England.

Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding processes.

Previous experience supporting adults in statutory or community settings.

Background in reablement, hospital discharge, learning disabilities, or mental health.

Experience in strength-based and person-centred practice.

Familiarity with local services, resources, and policy frameworks.
