What We’re Looking For

Qualifications & Registration:

Social Work degree or equivalent

Registered with Social Work England

Experience:

Prior experience in adult social care, ideally in hospital discharge, intermediate care, or reablement

Solid understanding of the Care Act 2014, Mental Capacity Act, and NHS Discharge to Assess processes

Skills:

Confident working in a multidisciplinary team

Strong assessment, planning, and time-management abilities

Experience with complex decision-making and working under pressure

Knowledge of Continuing Health Care (CHC) and NHS-funded care pathways

Familiarity with Mosaic or similar case management systems
