The Licensed Practical Nurse (LPN) reports to the Director of Care and/or designate. This role models leadership within an assigned community and is responsible for coordinating care delivery for the persons served. The LPN performs duties and monitors departmental activities according to their scope of practice, standards, regulations, policies, and procedures to maximize individualized, resident-centered care.

• Be an engaged, collaborative team player, developing and maintaining professional relationships that support and strengthen the organizational culture and purpose.
• Assess residents’ needs; implement and evaluate individualized resident-centered care plans.
• Ensure residents' rights are observed and adhere to a holistic philosophy of wellness.
• Liaise with physicians, community agencies, families, visitors, volunteers, other disciplines, and departments to ensure quality resident care.
• Assess and treat wounds, acting as a resource for care staff on skin breakdown prevention. Consult with RN or designate for treatment changes.
• Administer prescribed medications and treatments within the scope of the BC College of Nurses and Midwives (BCCNM).
• Monitor and report changes in residents’ physical and psychological conditions; document and communicate changes to families and physicians.
• Participate actively in Interdisciplinary Care Conferences and care plan reviews.
• Support administrative tasks such as admissions, discharges, and maintaining confidentiality.
• Participate in orientation, community programs, meetings, committees, and educational sessions.
• Provide coaching and direction to care staff, using in-the-moment education and completing Point of Care documentation; seek assistance from RN or designate as needed.
• Maintain up-to-date knowledge of community policies, especially regarding emergency procedures and equipment management.
• Perform other related duties as required.

• Graduate of an approved Practical Nursing program and hold a current license with BCCNM as an LPN.
• Valid Occupational First Aid Level 1 certification.
• Successful completion of annual Resident Assessment Instrument (RAI) competency testing.
• Previous experience in a long-term care facility is an asset.
• Pass the vulnerable sector Criminal Record Check per provincial guidelines.
• Proficient in reading, writing, and speaking English; able to understand verbal and written instructions.
• Computer skills including basic navigation of Word, Excel, Outlook.
• Ability to lift and carry up to 25 lbs and work with cleaning supplies and equipment.
