Our Visiting nurses provide client care in the client’s own home. They offer physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counselling to individuals and families, and more.
Job Summary:
The Registered/Licensed Practical Nurse is responsible for delivering and managing client- and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and community. They ensure care is provided within the framework of quality & risk management, adhering to the standards and scope of practice set by VON and the relevant provincial regulatory body. The nurse also provides safe and appropriate teaching, and delegates tasks to other care workers as appropriate.
Key Responsibilities:
- Provides nursing care to clients with less complex needs according to an established care plan; consults with an RN or other healthcare provider for more complex cases.
- Manages and evaluates care via established plans and responses.
- Delivers cost-effective, organized, outcome-oriented care.
- Conducts comprehensive assessments of clients and their environments using established procedures.
- Gathers relevant information from clients, families, and healthcare team members.
- Documents all relevant information clearly, accurately, and according to VON standards.
- Documents assessment findings and client/family reactions to care.
- Identifies problems and focuses care on key issues using established procedures.
- Contributes to care plans collaboratively with the healthcare team.
- Implements strategies from care plans in consultation with clients and caregivers.
- Documents and rationalizes modifications to care plans.
- Negotiates care delivery with clients and families, respecting their choices.
- Incorporates current research into clinical practice.
- Provides rationale for referrals, visit adjustments, and discharges based on sound data and guidelines.
- Utilizes tools and resources effectively and manages these resources.
- Delegates care to other workers and involves clients and families appropriately.
- Refers clients to appropriate care providers and disciplines following established guidelines.
- Promotes client-directed care, self-care, and independence through teaching.
- Shares clinical knowledge to empower clients, families, and the community.
- Uses adult learning principles and established materials for health education.
- Encourages family involvement in health decisions.
- Participates in developing health policies and nursing standards through involvement in VON, professional, and community organizations.
- Practices according to VON’s quality and risk management framework and Occupational Health and Safety legislation.
- Plans, implements, evaluates, and adjusts care based on client feedback to meet needs.
- Engages in decision-making and quality improvement activities.
- Follows VON policies, procedures, and strategic directions.
- Collaborates with management and colleagues to evaluate practice and maintains competence through ongoing education and self-reflection.
External and Internal Relationships:
- Works collaboratively within a multidisciplinary team, acting as a resource and mentor.
- Engages regularly with clients, families, and caregivers, providing education and support for independence.
- Participates in healthcare professional organizations and community activities to promote nursing best practices.
Education, Designations, and Experience:
- Must hold a license as a Registered or Licensed Practical Nurse in the province of practice.
- Additional degrees or certificates may be required for specialized roles.
- Clinical practice experience is an asset.
- Experience in community health is an asset.
