Overview

As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners. What will you do?
  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
  • Link patients with service providers
  • Coordinate and monitor care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected
What must you have?
  • Membership, in good standing, with the applicable regulatory body
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • College of Dietitians of Ontario
  • College of Audiologists and Speech Language Pathologists of Ontario
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • A valid driver’s licence and access to a reliable vehicle
  • Proficient in a Windows environment
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
What would give you the edge?
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language

What do I need to know?

Hours of work: 7.5-hour shift (tour) between the hours of 8:00 and 20:00. Ontario Health atHome is accessible seven days per week, including weekends and statutory holidays.

Availability Requirements

All new Part-time B hires are required to be available for 6 weeks of full-time (37.5 hours per week) orientation (fully paid).

To maintain Part-Time B status, availability is defined as:

  • Minimum five (5) shifts in a two-week period, with availability on those days from 08:00 to 20:00, not to exceed 37.5 hours per week. At least one day per week must be Monday or Friday
  • One weekend out of three
  • Availability for five (5) paid holidays in each fiscal year, including Christmas and New Year’s Day
  • Availability for forty-four (44) calendar weeks per year
  • No more than 3 weeks off during the summer period (July and August)
  • No more than 50% of PT B employees at any site may be unavailable in any one month; conflicts are resolved by seniority

Availability for Part Time B must be submitted in writing by the due dates shown in scheduling communications.

Care Coordinators may need to travel in the Chatham Kent area to meet and collaborate with patients and their families to develop individual plans for service provision.
