A regional health support organization is seeking a part-time Care Coordinator in Sarnia, Ontario. As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners. Hours of work: 7.5-hour shift (tour) between the hours of 8:00 and 20:00. Ontario Health atHome is accessible seven days per week, including weekends and statutory holidays.
Availability Requirements
All new Part-time B hires are required to be available for 6 weeks of full-time (37.5 hours per week) orientation (fully paid).

Care Coordinators may need to travel in the Chatham Kent area to meet and collaborate with patients and their families to develop individual plans for service provision.
