Job Summary

The Nurse II (Case Management) – RN, Specialist Services is a Registered Nurse who works in collaboration with team members including the unit’s specialists to support clients during their duration of their involvement with the service. The role provides nursing care and case management to out-patients. The Nurse II (Case Management) – RN, Specialist Services will support health evaluations, use clinical and case management skills, and work with clients with their individualized treatment plans. The Nurse II (Case Management) – RN, Specialist Services reports to the Manager – Specialist Services.

Qualifications
  • Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment).
  • Ability to communicate in one of the First Nations dialects in the Sioux Lookout Zone is an asset.
  • A bachelor’s degree in nursing (RN).
  • Registration and good standing with College of Nurses of Ontario with commitment for ongoing maintenance, or the ability to become registered.
  • Minimum of 2 years’ related experience is required, preferable in a mental health, medical or hospital setting.
  • Preferable experience is a nurse who has worked with individuals with pediatric populations and/or individuals with complex mental health issues, addiction issues, trauma, mental health diagnoses.
  • Demonstrated skill and competence to establish supportive trusting relationships with clients.
  • Experience with complex case conferencing and case management.
  • Strong verbal and written communication skills.
  • Strong computer and technology skills (or ability to quickly learn and adapt) are required due to electronic documentation and care planning requirements.
  • Ability to develop and maintain respectful, cooperative working relationships with physician services staff, other SLFNHA departments (Including Nodin), First Nations people, communities, community‑based workers, and health care providers.
  • Demonstrated effective problem solving and critical thinking skills.
  • Connections with and understanding of community services, resources, and supports is an asset.
  • Professional integrity and the ability to maintain privacy and confidentiality are essential.
  • Knowledge of the people, culture, and mental health priorities of First Nations communities in the Sioux Lookout region is an asset.
  • Ability to communicate in one or more of the First Nations dialects of the Sioux Lookout region and previous experience working with First Nation communities is an asset.
  • Ability to perform the requirements of the position on a regular basis.
Roles and Responsibilities
  • Collaborates and serves an inter‑professional team, applying nursing and case management expertise to support clients with their diverse health needs, including:
    • Education
    • Resource navigation
    • Care coordination
    • Advocacy
  • Support clients from the referral process, including admission, discharge and follow up functions.
  • Works with specialists to gather information, complete assessments, and implement recommended interventions.
  • Coordinates communication among clients, families, caregivers, and service providers to ensure client‑centered care.
  • Supports families/caregivers regarding treatment and recovery.
  • Develop working relationship with First Nation community services and support referral process as needed.
  • Connect clients and families with community health resources.
  • Participates in quality improvement.
  • Provides support and intervention to clients and families as required.
  • Maintains accurate and timely documentation in the Electronic Medical Records system and protects all confidential information.
  • Travel may be required occasionally but not extensive.
  • Other duties that may be deemed necessary by immediate Supervisor.
