Overview

Horizon Health Network in beautiful New Brunswick is looking for a Community Indigenous Patient Navigator to join our team. The ideal candidate will act as a resource and point of contact for Indigenous clients and their families, healthcare providers and, when appropriate, community members, to ensure the provision of care is culturally safe and client-centered. The navigator will facilitate access to services, assist during discharge planning, and, using a Two-Eyed Seeing approach, support traditional Indigenous ways of knowing and healing practices with evidenced-based quality care. The goal is to ensure Indigenous clients are accessing appropriate care from appropriate sources, and to increase client capacity and connections to be stable and supported.

Who You Are
  • One (1) to three (3) years’ recent related experience in Indigenous health or social programming
  • Experience advocating for clients
  • Ability to work in an evolving, fast-paced setting
  • Proficiency in Microsoft Office applications
  • Strong mediation skills and ability to resolve conflict
  • Exceptional interpersonal skills and ability to foster trusting relationships
  • Understanding of Indigenous healing practices and person-centered care; ability to conduct, assist with, or facilitate smudging ceremonies
  • In-depth knowledge of Indigenous Health programming at systems, community, provincial and federal levels
  • Knowledge of local Indigenous demographics, languages, cultures, customs, and traditions
  • Ability to speak Wolastoqey or Mi’kmaq is an asset
  • Knowledge of cultural safety and Two-Eyed Seeing in healthcare and ability to apply these concepts
  • Understanding of Canadian colonization and Indigenous systemic racism and its impact on health and wellness
  • Ability to facilitate and/or make referrals as needed
  • Applicants must be of Indigenous descent
  • Ability to work independently, be self-directed and make competent decisions in a complex environment
  • Ability to travel throughout the region as required
  • Physically able to perform assigned duties; valid driver’s license and use of a vehicle
Education Required
  • Undergraduate degree in a health or social sciences discipline preferred
  • Additional certifications in health (e.g., trauma-informed care, birth doula, death doula) or willingness to obtain
What You Will Do
  • Support the facilitation of a Two-Eyed Seeing approach to wellbeing and care of Indigenous clients across Horizon Health Network facilities
  • Visit and engage with Indigenous clients in community settings
  • Provide advocacy and mediation, assist with health literacy, and empower clients to participate in health decisions
  • Promote traditional cultural practices in health and healing by facilitating access to cultural, spiritual, and linguistic resources
  • Provide consultation and navigation support to Indigenous clients, their families, and their healthcare team
  • Facilitate formal and informal education sessions for Horizon staff and community members
  • Participate in case conference meetings and discharge planning for Indigenous clients returning home or transitioning to long-term care
  • Accurate documentation in patient records
  • Collaborate with community rightsholders and stakeholders to ensure seamless transitions during and from acute hospitalization; facilitate and/or make referrals as needed
  • Maintain awareness of Indigenous community resources
  • Develop written resources for patients and the healthcare team
  • Provide education sessions to staff, physicians and community partners on Indigenous Health
  • Contribute to programming related to Horizon and Indigenous patient health needs
  • Work with community partners and internal colleagues to promote cultural safety and address racism
  • Assist patients with navigating the complaints process and ensure appropriate follow-up
  • Develop and complete daily and monthly workload statistics for submission to the Indigenous Liaison Committee and internal Horizon Health departments
