Job description

The Position

The Client Navigator, under the direction of the STC Health Centre Manager, will be primarily responsible for assessing client needs and support systems, and assisting clients to reach their identified health goals. They will act as a resource to navigate social service and healthcare systems, ensuring clients have the supports they need to optimize their health and wellbeing.

The Client Navigator will work to improve access to internal and external supportive services for clients, enhance health outcomes by removing barriers to access, promote early intervention to mitigate the need for higher intensity support later on, and reduce health disparities by ensuring access to appropriate services where and when clients need them.
