  • Valid driver's license is an asset.

Job Summary

Identifies the needs of individuals supported and delivers supports and programs consistent with life plans in order to enhance their capacity to function in all aspects of community living.

Responsibilities
  • Identifying program needs of individuals supported, participating in the development and implementation of life plans, and assisting in monitoring achievement of personal outcomes.
  • Providing case management to individuals on the caseload, including appropriate personal and physical care, medication administration and crisis response, and advocating on behalf of individuals supported.
  • Coordinating care with families, medical and other professionals, and other service providers to address needs.
  • Providing life‑skills training and emotional support to individuals on a one‑on‑one or group basis and referring to professionals when necessary.
  • Maintaining documentation, monitoring finances, ensuring an appropriate standard of living, and assisting with locating housing for individuals supported.
  • Providing direction to part‑time staff as required.
Qualifications
  • Post‑secondary education focused on Social Services or Developmental Services is required; knowledge of healthcare procedures and mechanical equipment such as wheelchairs, lifts, G‑tubes, and other specialized equipment is required.
  • Post‑secondary education in a non‑related field requires a minimum of 1 year of related work experience.
  • Successful completion of internal legislated and mandated training is required.
  • Valid driver’s license is an asset.
