Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Coordinator, Specialized Programs demonstrates clinical expertise and works independently as a member of an interprofessional team within the framework of departmental regulations, program and clinical guidelines.

Working across Fraser Health on assigned initiatives, the Coordinator collaborates with a variety of clinical staff and caregivers to assist in the assessment, development, implementation and evaluation of new programs, standards and systems designed to enhance the care and coordination of clients in the hospital and community. Participates at the local, regional and provincial levels to assist in the development of related systems, programs and projects for the specialized area.

Responsibilities
  1. Participates in the development of new programs, services, clinical policies, procedures and standards of practice for the program in conjunction with the Manager, Clinical Services and the inter-professional team.
  2. Coordinates the development of policies and procedures, with other involved specialties, for the management and evaluation of clients triaged within the region’s programs.
  3. Applies relevant nursing research and evaluates effectiveness of specialized relevant programs, services, clinical policies and procedures to determine opportunities for quality improvement; recommends modifications and assists with implementation.
  4. Functions as a liaison to hospital and community staff to facilitate the development of collaborative and participative program systems, including injury prevention. Participates in the development, organization and implementation of integrated comprehensive program systems at a regional and provincial level.
  5. Develops collaborative working relationships with other programs and clinical service units to facilitate and support quality care and injury prevention.
  6. Establishes and maintains liaison with representatives of each department, as well as community stakeholders, to permit efficient communication regarding patient management problems.
  7. Identifies at risk populations for program clients and assists in developing prevention strategies with other programs.
  8. Coordinates and circulates appropriate program data for feedback and problem identification and resolution; makes recommendations as appropriate.
  9. Participates as a member on Fraser Health committees as well as ad hoc committees as required at the local, regional and provincial level.
  10. Reports unusual occurrences and risk management issues as it relates to the program.
  11. Performs other related duties as required.
Qualifications
Education, Training & Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three years recent related clinical experience in the specialty area including leadership experience, or an equivalent combination of education, training, and experience.

Valid Class V BC Driver's License and access to a personal vehicle for business-related purposes.

  • Knowledge of research processes and methodology.
  • Ability to operate related equipment including relevant computer applications.
  • Physical ability to perform the duties of the position.
