Qualifications

  • Social Services Diploma plus one year’s recent related experience or an equivalent.
  • Valid BC Driver’s License and access to personal vehicle.

Responsibilities

  • Provide support services to clients and families in community health.
  • Act as an advocate and liaison for clients.
  • Liaise and coordinate with medical professionals.
  • Support clinical staff by providing appropriate information.
  • Monitor clients and report observations to clinicians.
  • Assist in planning and organizing team-based care conferences.
  • Provide information on community services to clients.
  • Establish rapport and promote self-management behaviors.
