Candidates must have a post-basic qualification in chronic disease management and be a registered nurse with the Singapore Nursing Board. Qualifications
- Post-basic qualifications in chronic disease management are required.
- Experience in managing patients with chronic conditions is preferred.

Responsibilities
- Assess and deliver evidence-based nursing care for chronic conditions.
- Teach self-management techniques for chronic diseases.
- Monitor patient progress and coordinate with community partners.

- Navigate care systems and coordinate with community partners for continuity
- Monitor patient progression, track outcomes and recall defaulters
- Support chronic care initiatives and health education programmes
- Assist in developing clinical protocols and patient education materials
Requirements
- Post-basic qualification in Diabetes Management and Education / Gerontology / Chronic Disease Management. Other related post-basic qualifications may be considered.
- Bachelors Degree in Nursing will be an added advantage
- Experience in nursing and managing patients with chronic conditions is preferred
- Registered Nurse with Singapore Nursing Board
